Only one person per computer station needs to submit registration information. Two people per computer station may attend the class. However, only the person who's name appears on this registration form will be able to request a certificate of completion for attending the class and is able to submit for ASHA CEU credit. If you are registering more than one computer station, a separate registration with a different name must be completed for each computer station.
Please include the course name, date and time you wish to attend. Please be sure you are familiar with the equipment needed before attending class. All classes are taught in English.
