Right to Confidential Communications Policy

Effective date of policy: April 14, 2003

You may request to receive confidential communications of your protected health information (PHI) from PRC.

You may request that communications from PRC be sent to an alternate location or by an alternate means. PRC will accommodate reasonable requests for such confidential communications. You are not required to give a reason for this request.

PRC prefers these requests be in writing.
