Use the form below to request authorization to send your device to PRC for repair. A $62.50 fee will be charged if you send a device to PRC without an authorization number.

  1. Complete the information below, and click “Send Request”.

  2. We’ll email you with a Return Authorization Number (CA#) authorizing you to send the device to PRC within 1 business day.

  3. Once you’ve received your Return Authorization Number (CA#), you can send the device to PRC:

  4. If the repair will be paid for by a Insurance, Medicaid or Medicare, visit http://funding.prentrom.com to access the Funding Submission Assistant or Forms Locator to get started completing the paperwork.

Notes

  • Accessories
    • Accessories get misplaced or lost easily. Please don’t send PRC any accessories unless we request them.
    • If you send accessories to PRC and want them returned before your device, we’ll ask you to pay the shipping to have them sent separately.
    • Any accessories not claimed within 30 days of your device being returned are forfeited.
  • For a non-warranty repair, the cost of a loaner ranges from $135 - $300, depending on the device.
  • Any time a loaner is requested (regardless of warranty), someone must assume financial responsibility for damages to the loaner.
  • When you receive your CA#, use it to check the repair status at http://prentrom.com/repairs.
