Patient Access to File Information Policy

Effective date of policy: April 14, 2003

You have the right to inspect and receive copies of your medical records in our files. PRC may charge for the copying of the record, as well as supplies, labor, and postage. You will be notified of this cost in advance. You must agree to this financial responsibility in writing, in advance. (See form.)

PRC has the right to deny your request to inspect and copy the medical records in your file. This denial must be in writing and explain why the request has been denied.

There are several circumstances when the denial may not be appealed (unreviewable denial).

• Information compiled in reasonable anticipation of or for use in a civil, criminal, or administrative action proceeding.
• PHI regarding an inmate at a correctional facility.

You should make this request on the Authorization for Disclosure of Health Information form, which is then submitted to the privacy officer for action.
